Dear Dr. Seckel, Marionette lines - know what they are? Need to know if they are better treated with Botox or another injectable, which begins with “R”, have forgotten name. Thank you. Marcia, Maynard, MA

Dear Marcia,

Thank you for your question about Marionette Lines. They are those deep lines that form below the corners of your mouth and run down onto the chin. Many people complain that the Marionette Lines give the face an angry look. They are called Marionette Lines because they resemble the mouth of a puppet or Marionette. Correction of Marionette Lines is an important part of any facial rejuvenation program or skin rejuvenation or non -surgical facial rejuvenation program.

I use Restylane®, radiesse and juvaderm to treat Marionette Lines and Nasal Labial fold lines. The fillers Restylane, radiesse and juvaderm ar injected into the skin beneath the deep marionette line to plump the line. Filler injections are a camoflauge not a correction of the marionette and nasal labial fold lines.

I also use small amounts of botox to help lessen the Marionette line. The depressor muscles of the mouth pull down on the corners of the mouth and accentuate the marionette line and help make the jowl or laxity along the jawline more noticeable. By using small amounts of botox to weaken the depressor muscle, the Marionette line is lessened and the jowl is lessened. Using Botox® around the mouth is a very tricky technique and only small amounts should be used-otherwise the mouth can be weakened or distorted.

If you are considering this treatment I strongly urge you to see aboard certified plastic surgeon who is very experienced in this technique. This is not something you want the occasional user to do to your face. Fillers are an important technique for non-surgical facial skin rejuvenation, but there use requires skill and experience.
