Dear Dr. Seckel, I am scheduled to have a blepharoplasty to correct the dark circles and bags under my eyes. I asked my doctor about the laser blepharoplasty and he does not do the laser blepharoplasty. He says he doesn’t see any advantage, but he does not have a laser. What should I do? Thank you, Michelle, Boston,MA.

Dear Michelle,

As I say repeatedly in all my writings the MOST IMPORTANT thing for any patient considering eyelid rejuvenation surgery for removal of eye bags and dark circles under the eyes is to find a board certified plastic surgeon they trust, feel comfortable with, who has a good reputation, is very experienced in eyelid surgery and is certified by the American Board of Plastic Surgery. If you have enough confidence in your doctor to have scheduled  lASER blepharoplasty surgery then you should have confidence in that doctor and stay with him/her.

The choice whether to use a laser or scalpel to perform blepharoplasty eyelid surgery or eyelid rejuvenation for removal of bags under the eyes and dark circles under the eyes is up to the doctor. Both techniques produce good results when used by a skilled, experienced surgeon. The most important thing for you to do is to have your blepharoplasty done by a skilled, experienced surgeon. Let the doctor worry about which technique he/she is most comfortable with and can get the best result for you.

I would discuss with your doctor whether or not he/she thinks the operation will remove the dark circles under the eyes. In my experience an arcus marginalis release is required to remove dark circles under the eyes during blepharoplasty. I have written many posts about this issue, which you can find in the archives. I also like the laser blepharoplasty because it provides the opportunity for skin tightening and removing skin wrinkles by doing laser skin rejuvenation or laser peel of the eyelid at the same surgey.
