Hello Doctor. I have a few visible wrinkles under my eyes that I do not think will improve much using topical creams since some of them are really deep. Is there a laser treatment for this problem, or any other surgical procedure that will get rid of the wrinkles under the eye? George, Toronto, ON

Dear George,

I am happy to say that there are several new and very exciting non-surgical eyelid rejuvenation methods for effectively removing eyelid wrinkles. Excellent results are possible.

The most effective and quick eyelid rejuvenation method is laser skin rejuvenation treatment with the Erbium Laser called the microlaserpeel, although there is a 7-10 day period during which the eyelids are red following the procedure. Also if your lower eyelids are lax or droopy a surgical tightening procedure called a canthopexy must be done first because the laser skin rejuvenation procedure tightens the eyelid skin and can pull the eyelid down. It is very important that you consult a plastic surgeon with experience in laser eyelid surgery and eyelid rejuvenation if you are considering laser eyelid surgery.

There is another new less intense no down time procedure that also can improve eyelid wrinkles and rejuvenate the eyelid. If you do not want to have a period of redness there is a new procedure called LaserGenesis® using a laser made by Cutera® which is reported to smooth eyelid wrinkles. Unlike the Erbium laser peel or microlaserpeel procedure there is no redness or recovery time and minimal risk of the procedure pulling the eyelid down. You must however understand that the LaserGenesis® procedure needs to be repeated and the eyelid wrinkle removal takes time. The treatments are quick – 10-15 minutes, but you will need 5 or six treatments spaced a few weeks apart. If you have the patience and don’t want the downtime and risks associated with the Erbium laser procedure the LaserGenesis® procedure may be best for you.

The cost in my area for the Erbium Laser peel or microlaserpeel procedure is $2500 and for the Laser Genesis® procedure is $350 per treatment or $1750-2100.

Finally whichever procedure you choose to have done, once the eyelid wrinkles have been smoothed out it is important to take steps to prevent them from coming back. This means following a healthy diet and lifestyle, using a good prescription topical cream such as Retin A or Vitamin C, and Botox® injections to quiet the muscles that cause Crow’s Feet and eyelid wrinkles in the first place.
