Dear Dr Seckel, I am a 27 year old man and i am considering doing a canthoplasty or blepharoplasty procedure on my eyes because of the tired look that they give me… it has been like this forever but i always get asked questions like are you ok and you look tired.i also have abit of puffiness and dark circles that would not go away with anything. my main concern however is the way my eyes will look and if this will change this dropping effect. what in your opinion are the main disadvantages and implications?which is the most appropriate procedure that i should follow? and also what is your opinion on buttocks implants on men? thank you very much! The eyelid rejuvenation procedure called canthoplasty or canthopexy is an eyelid surgery that is most often done to correct ectropion the downward droop of the outer portion of the lower eyelid which often occurs with aging or in patients who have had a previous blepharoplasty through an external incision. The ectropion is also called scleral show, sad eyed look, or hound dog eyes. After the canthopexy or canthoplasty the eyes will be slanted up at the corners but usually will return to normal after about six weeks. It is a very good procedure if you have ectropion or lax lower eyelids that are drooping at the outside corners and turning down to give you a sad eyed look, hound dog eyes, or scleral show.

Generally when people complain of a tired look they are referring to dark circles under the eyes and puffiness or bags on the lower eyelids. If this is the case with you then an eyelid rejuvenation procedure called a blepharoplasty not a canthopexy or canthoplasty is what you need. The blepharoplasty, which I do with a laser through an incision on the inside of the lower eyelid (transconjunctival laser blepharoplasty approach), removes fat and tightens the skin and removes the tired look.
