Lipstick lines can be improved by weakening the muscle with Botox®-

Dear Dr. Seckel, Wow, it is refreshing to see such a well-informed site! It seems I have, ‘Naso-jugal Fold or Tear Trough Deformity-the True Dark Circle Under the Eye’ to use your words. I am not sure if you cover this topic under blepharoplasty in your book which I just ordered, but why are the fillers, namely, Restylane not one of your favorites under the eyes? If no vision problems are first observed, are there possible long-term risks to the eyes? Are there other substances, within Restylane beside Hyaluronic Acid that are dangerous to the eyes in your opinion? Many thanks. Michael, NJ.

Worse, because fillers are easy to use, many doctors with no experience in eyelid surgery are tempted to use them, out of ignorance or lack of concern

Dear Dr. Seckel, Please tell me how to successfully treat my skin. I have acne, blackheads, whiteheads, redness and puffiness around my nose and huge pores around my nose and on my forehead. I’ve tried all types of products and prescriptions that work for about a month and then my skin seems to start resisting the product. I’m desperate at this point. I’m only 29. Is microdermabrasion right for me? What home microdermabrasion kit would you recommend? Thank you, Milly, Harrisburg, PA.

Dear Milly, For correction of enlarged pores, acne and acne breakouts I recommend you see a medical aesthetician in a Dermatologists or Plastic Surgeons office and have a professional microdermabrasion-at least 4 to start. I would also recommend Brevoxyl washes to reduce skin oil and a topical Retinoid-Differin or RetinA which will require a prescription. [...]

Dear Dr. Seckel, What is the safest most effective way to firm or tighten facial skin of a (not too saggy) face without radical facelift surgery? And do you know someone who does this close to Spokane? Thank you, Diana, Spokane WA.

several non-surgical facial rejuvenation procedures available today that will tighten facial skin without the need for face-lift

Dear Dr. Seckel I am a Canadian living in working in Asia. To get to the point I have dark (purple color) under my eyes closer to the nasal side. However, the darkness does not extend across the entire area under each eye. It goes down more than over. I would say over almost halfway. Likewise, the skin area in the ‘pocket’ where the upper eyelid and the nasal/forehead area meet is purple/dark. I really want to eliminate this as it is getting to the point where I avoid mirrors. I have had some laser treatment here recently. I am not sure of the procedure’s name, but to explain it a laser ‘zapped’ me (hurt too) a several times in the problem areas. I was told this was to promote collagen development and help to reduce the color intensity. However, after two sessions (a month apart, as suggested), I haven’t seen a result. Would a blepharoplasty help? I will be going to Toronto in April, and if a remedy is in sight I would be willing to travel anywhere necessary during that time. Thank you, Michael

Dear Michael, The dark purple eyelid skin is caused by the color of the muscle underneath the skin showing through, is normal and cannot be corrected with a laser skin eyelid rejuvenation technique. However the dark purple color at the bottom of the lower eyelid near the nose which creates dark circles under the eyes [...]

Dear Dr. Seckel, I am interested in Blepharoplasty or some form of eyelid rejuvenation or eyelid tuck. I have read that Blepharoplasty is a safe operation and that Blepharoplasty can be very effective for removing eyelid hooding and eyelid bags. I also have dark circles under my eyes and want to know if Blepharoplasty can remove the dark circles. I have read that Blepharoplasty can be done with the scalpel and with the laser but opinions vary widely about which Blepharoplasty method is better. Can the Blepharoplasty be done under local anesthesia? What is the recovery time after a Blepharoplasty? What are the risks of a Blepharoplasty? Can the eye itself be injured during a Blepharoplasty? Is there such a thing as a non-surgical blepharoplasty? Can filler like Restylane be used to get the same benefit for dark circles instead of a Blepharoplasty? Does the non-surgical facial rejuvenation program you speak about rejuvenate the eyelids and make a Blepharoplasty unnecessary? I am 42 and have early eyelid hooding of the upper eyelids and puffiness and bags on the lower eyelids. I am beginning to form a dark circle under the eyelid near the nose-in your book you call this the naso-jugal fold or tear trough deformity. Will the Blepharoplasty correct the tear trough deformity? Thank you very much, Charlene, Boston, MA.

blepharoplasty can be safely performed by a skilled, experienced board certified plastic surgeon.

Dear Dr Seckel, I am a 27 year old man and i am considering doing a canthoplasty or blepharoplasty procedure on my eyes because of the tired look that they give me… it has been like this forever but i always get asked questions like are you ok and you look tired.i also have abit of puffiness and dark circles that would not go away with anything. my main concern however is the way my eyes will look and if this will change this dropping effect. what in your opinion are the main disadvantages and implications?which is the most appropriate procedure that i should follow? and also what is your opinion on buttocks implants on men? thank you very much! Ian, London, UK.

Dear Ian, The eyelid rejuvenation procedure called canthoplasty or canthopexy is an eyelid surgery that is most often done to correct ectropion the downward droop of the outer portion of the lower eyelid which often occurs with aging or in patients who have had a previous blepharoplasty through an external incision. The ectropion is also [...]

Dear Dr. Seckel, I am 42 years old and in great shape but people tell me I look tired. I have eyelid bags and dark circles under my eyes and have read about blepharoplasty. Is there any non-surgical way to remove eyelid bags? A friend of mine had a blepharoplasty done and I can see the scar. Thank you, Denise, Boston, MA.

Dear Dr. Seckel, Would you recommend silicone as a safe and effective filler for the naso-labial area? What about Perlane? Is it effective? Thank you. Diana, McLean, VA.

Sunday, March 19th, 2006

I do not feel silicone is safe for injection in the skin-it is not lawful to use it unlesss

Dear Dr. Seckel, I have large pores and dents on my nose and large pores on my cheeks due to aging. I’ve tried many different products, but they were not working. Please let me know if there anything I can do to minimize these pores on my face. Thank you. Sunny, Las Vegas, NV.

Sunday, March 19th, 2006

Dear Sunny, The enlarged skin pores are caused by the atrophy or loss of Collagen in the deeper skin layers that occurs with facial aging. The enlarged pore is the opening to the skin oil gland which lies deeper in the skin in the dermis. New non-surgical facial rejuvenation techniques attempt to stimulate new collagen [...]
