Dear Dr. Seckel, I’m 44 years old. Thanks to my mother’s genes I’m aging very well. I was doing cooltouch laser for facial rejuvenation the last 4 years (6 month intervals).I don’t need wrinkle removal, but lately I’ve seen small sagging line from my nose to mouth.(”marionette lines”). I certainly don’t need a face lift but I am thinking to switch to Luxir skin tightening. Do you think it will be helpful in my case? Any other suggestions? Kate, San Diego

Dear Dr. Seckel: I am 41 years old and currently get Botox injections around my eyes for crow’s feet and Juvaderm around my mouth for nasal labial folds, about 2 times per year. I am very much interested in doing something a little longer lasting. My doctor recommended Active FX. What do you think would be best for me Active FX or Thermage? My main concern is the lower area of my face to just under my chin, which is just starting to sag. I believe you call this jowls and early platysmal bands or turkey wattle. This area really bugs me and I would love something that lasts longer than a few months but without a face-lift or plastic surgery. Any suggestions? Thank you, Lynda, Scottsdale,AZ

Dear Dr. Seckel, I am considering getting a Thermage skin tightening treatment. What results and risks can I expect with this procedure? I am hoping to correct sagging mouth, chin and jaw area, the jowls mainly. Both the LuxIR Skin Tightening machine made by Palomar and the Aluma with FACES made by Lumenis are excellent skin tightening devices.

Dear Dr. Seckel, I’ve purchased your terrific book and love it. I found a doctor in Los Angeles who performs skin tightening with the LuxIR system, but was told it cannot be used on the eyes. Sagging brows, slightly sagging eyelids, and the beginnings of jowls are my main concerns, and I do not want a blepharoplasty , face-lift or brow lift.. I’m curious why Thermage is used on the eyes, but not the IR systems, and if it would be my best bet. Will you please make any other suggestions to firm up eyelids and brows? I’d prefer to try a firming treatment and see those results before committing to a face-lift and blepharoplasty. Second question - I’ve also looked into Aluma Faces, but was told the Aluma system does not go as deep into the Dermis and may not be as effective as other RF or IR treatments. and there is a new dermal heating system called Affirm by the makers of Fraxel that I’m wondering if you can comment on, or suggest the latest and greatest this month in your opinion. Dear Dr Seckel I am 52 yrs with no skin problems except dry skin, however I do have wrinkles on forehead, crows feet and the deep line from nose to mouth (nasal labial fold. I do not want a face-lift but am looking for a procedure to take care of the above without going under the knife or having plastic surgery. I have seen some adverts for Titan, Fraxel and Thermage but with so many different treatments on offer, I do not know what would be best for me. Botox is the best and most effective treatment for forehead lines and crow’s feet.

Dear Dr. Seckel,All the skin tightening and non-surgical facial rejuvenation treatments confuse me. I am 45 years old and look about 35. I don’t want a face-lift. I have some wrinkling and looseness on my neck, some collagen breakage (this is what I’ve been told the lines are), some tension lines in my forehead, crows feet around my eyes and a little of the fold thickening around my mouth. My biggest concern is my neck. I have had microdermabrasions and there have been no changes. Is there anything that truly works consistently? In one of your answers I see another skin tightening treatment compared with titan in that it uses infrared. You stated you liked this the best for yourself personally. I have not heard of this before. Do either infrared treatments or radio frequency treatments cause damage? If so, does one cause more than the other? Thermage is RF and has had problems. There is a class action lawsuit against Thermage

Dear Dr. Seckel. I’m 55 and have some moderate sagging on my lower face and jowls. About 3 years ago I had one Thermage treatment in the hope of tightening that area. If there were results, they’re too subtle for me to discern. In the past 9 months, I had 2 Fraxel treatments seeking a tightening of that area. Again, no visible results. I just contacted a physician about having a Titan treatment for the same area because I’ve read that the down time (with redness and swelling) is less than Fraxel. The doctor is dubious about getting results since the two other similar treatments seemed to have little effect. While in his office, however, I was told about Sculptra and shown photos (from the manufacturer and one in-house patient) that would indicate that several Sculptra treatments might achieve the visible tightening I’m looking for. Would you please discuss Sculptra on your site in relation to skin-tightening? Also, if I decide to do it, how can I prevent or minimize bruising from the injections? I bruise very easily and being a workingwoman, I need to minimize “down time”. Thank you, Anita, Salt Lake City, UT

Certainly filler such as Sculptra is not the answer for tightening skin.
