Dr. Seckel, I am 40 years old I recently visited 3 plastic surgeons for consultations on blepharoplasty. I have bags and dark circles under my eyes and look tired all of the time. Two of the three surgeons wanted to do a traditional blepharoplasty with the incision on the outside of the eye where the fat pads are removed and they both said they would have to snip a minimal amount of skin. The third surgeon had a different approach. He wanted to do the blepharoplasty without removing the fat pads. I am sort of confused by all of the information he gave me but it sounds like he would do all of the tucking of the muscle from the inside. He also suggested an arcus marginalis release procedure to help with the dark circle in my naso-jugal fold. He wanted to use fat from underneath my chin for the arcus marginalis release procedure but said I didn’t have any fat in my neck/chin area. He also suggested a cheek suspension. I am just starting to notice a minimal droop in my cheek area. It is an endoscopic procedure is what I believe he said. I am thoroughly confused. I read your article regarding the feather lift, thread lift and Russian lift. Is the procedure he suggested the same thing? I don’t have a lot of wrinkling in my skin, mostly fine lines. I want to the minimum amount of surgery for the maximum effect. I also don’t want to change the way my face looks, I just want to get back a little bit of what I started with. Younger and refreshed. I do not recommend thread lift etc-they are not accepted techniques with proven longevity yet

Dear Dr Seckel, 2 years ago I had a lifestyle lift at the age of 42. The results were wonderful in the beginning but I was left with horrible scarring. I understood that I would have a scar in front on my ear, but I have huge scars 1/4″ below my ear that is thick and jagged. I look like I was in a car accident. I cannot wear my hair back or up and I can’t leave the house without self tanning lotion and heavy makeup. I wrote to the Dr. that developed this procedure but he has failed to respond. All I want to do is have this fixed, but I am concerned that this will be very difficult and expensive. Do you think I will have to have a full facelift to repair the scarring? Have you had any emails similiar to mine? Thank you for your time.Kelly, Jacksonville, FL

Dear Dr. Seckel, What is the safest most effective way to firm or tighten facial skin of a (not too saggy) face without radical facelift surgery? And do you know someone who does this close to Spokane? Thank you, Diana, Spokane WA.

several non-surgical facial rejuvenation procedures available today that will tighten facial skin without the need for face-lift

Dear Dr. Seckel, I am considering Thermage Face lift but was wondering in general what the price range is from lowest it could cost to highest. Thank you, Bette, Burlington, MA.

Dear Bette,
The Thermage is a skin tightening machine, can cost $4000 a treatment and needs to be done 3 times-the second and third treatments can cost 500-2000. Thermage is painful and results are inconsistent and can take 1 1/2 hours. Skin tightening is the newest non-surgical facial rejuvenation program available. It is not a face [...]

Dear Dr. Seckel, I AM 54 YRS OLD AND THE PAST FEW YEARS THE SKIN ON MY NECK HAS BECOME QUITE LOOSE, IS THERE A NON SURGICAL PROCEDURE FOR THIS PROBLEM, OR IS A FACE LIFT NECESSARY TO FIX THIS PROBLEM ELLEN, REVERE MA.

Dear Ellen,
Thank you for your question.
There are several new non-surgical facial rejuvenation skin tightening technologies available that can be used to tighten neck skin. Thermage®, Titan®, LuxIR, Refirme, and the newest one Aluma® with FACES.
Generally these non-surgical facial rejuvenation techniques get about a 30% skin tightening. They are not a non-surgial face lift. They [...]

Dear Dr. Seckel, What is the difference between facial rejuvenation and a face-lift? I am 50 and in great shape. But when I look in the mirror I see some sagging skin under my chin and dark circles and some puffiness under my eyes. I also have some brown spots on my cheeks and some wrinkles especially around my eyes. I want to look better but am not sure what I need. I always thought I would have a face-lift when the time came, but there are so many options advertised it is confusing. Thanks, Denise, Boston, MA

Dear Denise,
To rejuvenate means to make young or youthful again or restore to an original state. Facial rejuvenation or skin rejuvenation requires that the face or skin be restored to a young, youthful, or original state.
To understand facial or skin rejuvenation you must understand what facial aging changes have occurred on the facial skin and [...]

Dear Dr. Seckel, I am planning to have a face lift but have heard about Thermage. I asked my plastic surgeon but he does not do Thermage. My main problem I want to correct is that my neck sags like a turkey neck and I have bad jowls. Will Thermage help me? Thanks, Wanda, Boston, MA.

Dear Wanda,
I cannot give you a recommendation without examining you. If your plastic surgeon is a board certified plastic surgeon and you trust him/her then you should follow his/her advice.
The non-surgical facial rejuvenation skin tightening machine called Thermage® and the other new skin tightening technologies Titan®, LuxIR, Refirme and the Aluma® FACES®, can tighten skin [...]

Dear Dr. Seckel, I live in Boston and have seen several doctors about the loose skin on my neck. I have received different opinions about what to do. One doctor wants to do a face-lift. Another suggested I try Thermage. I have also read about the thread lift and lifestyle lift. I am 45 years old have good skin but am concerned that my neck and jaw line are starting to sag. Can you help me understand the difference between these procedures? Thanks, Meg, Boston, MA.

Dear Meg,
It is impossible for me to make a personal recommendation for you without examining you in person. It is very important that you seek consultation with an experienced board certified surgeon who can offer you all the options and choose the one that is best for your face. Avoid having someone “fit your face” [...]

Dear Dr. Seckel, I am 40 years old and am beginning to see some loose skin on my neck and a little bit of looseness on my jaw. My skin is good, I don’t have many wrinkles. I really don’t want a face lift. I have read that there are laser face lifts called non-surgical face lifts. Do these work well on the neck? Am I too young to have a face lift? Thanks, Claire, St Louis, MO.

Friday, January 27th, 2006

Dear Claire,
The early skin aging changes you see on your neck are very common and are often the first sign of facial aging that prompts people to consider some type of facial rejuvenation. Fortunately for you, you just have early skin laxity and don’t have many wrinkles, thus non-surgical facial skin rejuvenation will be more [...]

Dear Dr Seckel, My doctor has recommended that I have Thermage to tighten my neck. I have also heard of Titan and something called Faces. What do you recommend? Thank you, Myra, New York, NY.
