Dear Dr. Seckel, I am 38, fair skinned and thanks to my parent’s genes still look much younger than my age. My eyelids are starting to change and look older than the rest of my face. The upper eyelids are starting to droop and form hoods over my eyes and I am starting to have dark circles and bags under my eyes. I am using the Obagi products and have tried the Teamine complex but I really don’t see any improvement. I don’t want blepharoplasty or plastic surgery. I have read about a non-surgical blepharoplasty online. Does it work and how is it done, Thank you, Katie, Boston, MA.

Dear Katie,

The eyelids are the first part of the face to show signs of facial aging in almost all people. In many people, especially people with fair skin the eyelids start to show aging changes as early as the late 20’s. The first signs of eyelid aging are laxity or loose skin of [...]

Dear Dr. Seckel, Recently I saw a web cast describing a non-surgical blepharoplasty using Aluma FACES on the eyes (http://www.nbc4.com/health/9842667/detail.html). The video even showed the dr. using the wand directly on closed lids of a patient (over the eyeballs). The claim made is that this new technology is safe for use in this is manner while other lasers are not. Is this true? Many Thanks, Terry Ann, Kansas

I would certainly not do this and I have been using lasers for 12 years on the eyelids but only with a protective shield over the eyes.

Dear Dr. Seckel, I have malar bags (festoons) one of which is at least twice as big as the other. I have been told that a blepharoplasty will not correct festoons. Do you know of any treatment to help improve this? I’ve been told direct excision is the only way to correct this, but due to the scar factor, no one I have consulted with is willing to do it. I would appreciate your expertise on this matter. Thank you in advance. Paula, Windsor, ONT.

The most common complaint about festoons arises after a blepharoplasty has been done to remove puffiness or bags from the lower eyelids

Dear Dr. Seckel, I just had evolence, a soft tissue filler like Collagen injected under my eyes because I wanted to avoid a blepharoplasty. I now have lumps and bruising. I’m sure the bruising will go away, but the lumps look worse as if I have folds under my eyes. After three days the doctor injected a slight amount of cortisone and said that it is probably swelling, he’s never seen this before. I am scared I don’t know what I did; I was hoping that my eyes would look 50% better. I had Restylane around my mouth, very slight difference, but no lumps. Shouldn’t this doctor know what to do if the lumps are actually from the evolence that he used? Thank you, L, Toronto, Canada

Injecting anything around the eyelid is dangerous, even board certified Ophthalmologists are very careful about injecting around the eye.

Dear Dr. Seckel, Would it make sense to see an ocular plastic surgeon for blepharoplasty surgery and fillers around the eye area. After consulting with cosmetic surgeons for facelift procedures, many seem to get insulted when I tell them I will be seeing an ocular surgeon to address my eye rejuvenation issues. Thank you, Karen, Boca Raton, FLA

many doctors who call themselves “cosmetic surgeons” are not surgeons at all but dermatologists. There is no “board of cosmetic surgery” and a dermatologist is a skin doctor not a plastic surgeon.

Dear Dr. Seckel, I am a 32-year-old male. I have hollowness under my eyes and would like to use Restylane to fill the area under my eyes. > > The doctor says that he will also inject Botox to prevent the lower eyelid > muscles from moving during the recovery period of 3-4 weeks. > > I have read a few of the responses here and you are against Restylane > under the eyes. > > Why exactly? Thank you, Sheila, Calgary, CN

It is not whether but a matter of when someone will be blinded by an inexperienced, non-appropriately trained person injecting a filler into the eye

Dear Dr Seckel, I am a 27 year old man and i am considering doing a canthoplasty or blepharoplasty procedure on my eyes because of the tired look that they give me… it has been like this forever but i always get asked questions like are you ok and you look tired.i also have abit of puffiness and dark circles that would not go away with anything. my main concern however is the way my eyes will look and if this will change this dropping effect. what in your opinion are the main disadvantages and implications?which is the most appropriate procedure that i should follow? and also what is your opinion on buttocks implants on men? thank you very much! Ian, London, UK.

Dear Ian,
The eyelid rejuvenation procedure called canthoplasty or canthopexy is an eyelid surgery that is most often done to correct ectropion the downward droop of the outer portion of the lower eyelid which often occurs with aging or in patients who have had a previous blepharoplasty through an external incision. The ectropion is also called [...]

Dear Dr. Seckel, I am 52 years old and I had a consultation with a Cosmetic Plastic Surgeon to have a facelift, along with Erbium peel done on my lower eyelids and botox. Since the procedures are very expensive, I was wondering if you would recommend having the lower eyelids down with the laser and the botox and waiting to have the facelift, or go ahead and wait to have it all done at once, which is what he wants to do? From what I have read, the downtime may be high for the laser treatment, compared to the facelift. Thank you, Jenny, Salem, OR.

Dear Jenny,
I always recommend to my patients that they take their time in deciding to have a major facial rejuvenation procedure. I also tell my patients frequently to do smaller non-surgical facial rejuvenation options and save their money for expensive surgery later on. I don’t think people should spend money they can’t afford on cosmetic [...]

Dear Dr Seckel, Yours is the most informative site on eyes I have seen. I had an upper lid blepharoplasty, skin excision only, and there is a scar on the outer aspect of both eyes. I had fat injections to the malar crow’s feet. I’m not sure if this scar is the access area the surgeon used but I now have lower eyelid descent – and degree of ectropion which is frightening. I’m wondering if fat was removed as well. Please help me if you can I would like to come and see you. Thank you. Linda, London, England.

Dear Linda,
Thank you for your question. I am so sorry to hear of the problem you have suffered following your eyelid rejuvenation blepharoplasty and fat injection but would reassure you that techniques are available to improve your scars and correct the ectropion. The most important thing for you to do is to seek consultation with [...]

dear dr seckel i am writing to you to gain your opinion about areas of my appearance i am unhappy with. first, i have some darkness/hollowing below my eyes. also, some fine wrinkling above. second, i had a chin implant operation a few years ago. although i like the augmentation provided, there is some scarring/discoloration around my chin/jaw area. Please find attached photos to try and illustrate. Are there procedures available to provide an improvement? I look forward to hearing from you. kind regards, Toni
